This document provides resources and a suicide risk screening tool for patients. The resources include the National Suicide Prevention Lifeline and Crisis Text Line, which are available 24/7. The screening tool consists of five questions that assess a patient's suicidal thoughts and behaviors. If the patient answers "No" to questions 1 through 4, no intervention is necessary. If the patient answers "Yes" to any of these questions or refuses to answer, they are considered a positive screen. Question 5 assesses the patient's current thoughts of killing themselves. If the patient answers "Yes" to this question, they require a STAT safety/full mental health evaluation and cannot leave until evaluated for safety. The patient should be kept in sight and all dangerous objects should be removed from the room. If the patient answers "No" to question 5, they require a brief suicide safety assessment to determine if a full mental health evaluation is needed. The physician or clinician responsible for the patient's care should be alerted in both cases.
